Ways to Get Your Funding Denied:

The following 5 mistakes are typical errors that get many applications denied:

  1. Sign a contract less than 28 days after your Form 470 filing date.
  2. List a service provider as a contact on your Form 470.
  3. Submit a funding request for a service not listed on your Form 470.
  4. Miss the Form 471 filing window.
  5. Fail to certify a Form 470 by the closing window date for your Form 471.

Please remember that we need the Letter of Agency form completed each year by the Superintendent or Business Administrator of a district in order for them to be included in our consortium application. Your district also needs to be CIPA compliant and to submit Form 479s to the NERIC when requested each year. Your district’s Technology Plan needs to include the year of the consortium application as well. If the NERIC is missing a Letter of Agency form, a Form 479 or a current Technology Plan, E-rate eligible expenses cannot be reimbursed from our consortium application to your district.

And don't forget the FCC requires a five year document retention for your E-rate paperwork.  This includes not only E-rate forms, but all supporting information such as technology plans, discount rate support, competitive bids received, bid selection results, inventory records, invoices, and payments.
